Sacramental Preparation (Enrolment Form)

Since the Diocesan Synod in 1992, all children are to be prepared for the Sacraments by their parents in a Parish-based program organised by the Parish Sacramental Team.

In the Catholic Parish of Forster Tuncurry, we have been preparing children this way for over twelve years and have found it a very warm and welcoming program. No longer are schools responsible for preparing children for the Sacraments, however we do have a Sacramental Team made up of teachers and parents. The role of educating children in Faith has been given back to where it rightly belongs – with you as parents. The Sacramental Team will provide the required support to parents in this regards.

What Do I Do Next?

There are a number of things that we would ask you to do if you would like to enrol your child in the Sacraments of Initiation program:

Enrolment Form

Please complete and submit the Enrolment Form below so that we can register your interest in preparing your child for the Sacraments of Initiation.

Baptism Certificate

For your child’s preparation, we need a recent copy of their Baptism Certificate. This can be emailed to the parish office: forster-tuncurry.parish@mn.catholic.org.au

If your child was baptised in Forster Tuncurry, please indicate this on the Enrolment Form as we do not need to sight the certificate as we can look it up in our own records. If, however, your child was baptised in another Parish, please complete the ‘Copy of Baptism Certificate Form’ and send it to the Parish in which your child was baptised.

Allergies

It would be appreciated if you could advise a member of the Sacramental Team of any allergies your child suffers from that might be relevant to the celebration of Confirmation and First Communion. In particular, children who have been diagnosed with Coeliac Disease will need to have special arrangements made to obtain low-gluten hosts in time for the celebration of their First Communion, along with the appropriate permission of the Bishop.
